To ensure the safety of all dogs in our care, the following vaccinations are required.

Submitting Vaccination Records

Vaccination records must be provided prior to your pet’s first visit. You may:

• Upload records directly to your client portal

• Email your pet's records to doghouse@shots.pet

• Fax your pet's records to (866) 629-1801

• Bring a paper copy of your records to our lobby

• Ask your vet to email or fax your pet's records to us directly

Important Notes

• Vaccines must be current and administered by a licensed veterinarian

• We are unable to accept pets without approved vaccination records

• It is the client’s responsibility to keep records up to date
